Post-Acute Care Nurse Practitioner or Physician Assistant Wakefield, RI (Full-Time)

We are hiring a Nurse Practitioner or Physician Assistant to join the Post-Acute Care team in Wakefield, RI (additional openings in Wakefield, RI and North Conway, NH). This full-time, facility-based role focuses on caring for patients transitioning from acute hospital settings to skilled nursing and rehabilitation facilities.

Key responsibilities

• Perform facility-based rounding in post-acute care settings.
• Evaluate and manage acute and chronic medical conditions.
• Develop and implement care plans, coordinate transitions of care, and communicate with interdisciplinary teams, families, and outside providers.
• Monitor patient progress, adjust treatment plans, and facilitate timely interventions to reduce hospital readmissions.
• Document clinical encounters and maintain accurate records in the EMR.
• Provide patient and family education and support long-term relationship-building with patients and care teams.

Schedule and work environment

• Full-time, Monday–Friday, 9:00 AM–5:00 PM.
• No weekends, no on-call responsibilities.
• Autonomous clinical practice with established clinical and operational support.
• Team-based collaboration with nurses, therapists, social workers, and facility staff.

Compensation & benefits

• Base salary: $120,000 per year.
• Total earning potential: $180,000–$220,000+ (productivity/bonus potential).

Required qualifications

• Graduate of an accredited NP or PA program.
• Current, unrestricted NP or PA license in the applicable state or eligible for licensure.
• National board certification (NP or PA).
• DEA registration or eligibility to obtain.
• Minimum one year clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred.
• Strong clinical assessment, diagnostic, and care coordination skills.
• Excellent communication and interpersonal skills; able to work independently and collaboratively.
• Proficiency with EMR systems.
