Job Description
We are hiring a Nurse Practitioner or Physician Assistant to join the Post-Acute Care team in Wakefield, RI (additional openings in Wakefield, RI and North Conway, NH). This full-time, facility-based role focuses on caring for patients transitioning from acute hospital settings to skilled nursing and rehabilitation facilities.
Key responsibilities
- Perform facility-based rounding in post-acute care settings.
- Evaluate and manage acute and chronic medical conditions.
- Develop and implement care plans, coordinate transitions of care, and communicate with interdisciplinary teams, families, and outside providers.
- Monitor patient progress, adjust treatment plans, and facilitate timely interventions to reduce hospital readmissions.
- Document clinical encounters and maintain accurate records in the EMR.
- Provide patient and family education and support long-term relationship-building with patients and care teams.
Schedule and work environment
- Full-time, Monday–Friday, 9:00 AM–5:00 PM.
- No weekends, no on-call responsibilities.
- Autonomous clinical practice with established clinical and operational support.
- Team-based collaboration with nurses, therapists, social workers, and facility staff.
Compensation & benefits
- Base salary: $120,000 per year.
- Total earning potential: $180,000–$220,000+ (productivity/bonus potential).

Required qualifications
- Graduate of an accredited NP or PA program.
- Current, unrestricted NP or PA license in the applicable state or eligible for licensure.
- National board certification (NP or PA).
- DEA registration or eligibility to obtain.
- Minimum one year clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred.
- Strong clinical assessment, diagnostic, and care coordination skills.
- Excellent communication and interpersonal skills; able to work independently and collaboratively.
- Proficiency with EMR systems.
