Job Description
Join our growing Post-Acute Care team on Cape Cod, MA, providing high-quality, patient-centered care to individuals transitioning from hospital to skilled nursing and rehabilitation settings. This full-time, facility-based role emphasizes continuity of care, reduction of hospital readmissions, and collaborative management of chronic and acute conditions alongside interdisciplinary teams. Additional openings are available in Wakefield, RI and North Conway, NH.
Key responsibilities:
- Perform regular facility rounds in post-acute care settings
- Assess, diagnose, and manage acute and chronic medical conditions
- Coordinate care plans with nursing staff, therapists, and families to support recovery and optimize outcomes
- Provide patient and caregiver education and advance care planning
- Document thoroughly in the EMR and participate in quality improvement initiatives
Schedule and work environment:
- Full-time, Monday–Friday, 9:00 AM–5:00 PM
- No weekends or on-call duties
- Independent clinical autonomy with strong operational and team support
- Opportunity to build long-term relationships with patients and care teams
Compensation and benefits:
- Base salary: $120,000 annually
- Total earning potential: $180,000–$220,000+

Qualifications:
- Graduate of an accredited NP or PA program
- Current, unrestricted NP or PA state license or eligibility for licensure
- National board certification (NP or PA) and DEA registration or eligibility
- Minimum one year clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred
- Strong clinical assessment, diagnostic, and care coordination skills
- Excellent communication and interpersonal skills; ability to work independently and collaboratively
- Proficiency with EMR systems
