Position highlights
- Monday–Friday schedule; no nights or weekends
- Small patient panels (8–12 patients/day) to support thorough, relationship-based care
Compensation & benefits
- Base salary $165,000–$185,000
- Up to 20% performance-based bonus

Requirements
- Active Nurse Practitioner or Physician Assistant license in New York
- Minimum 2 years of primary care experience
- Experience or strong interest in geriatrics preferred
- Excellent clinical judgment, communication, and organizational skills
- Medicare provider ID required
