Float Family Medicine Nurse Practitioner (FT) — Phoenix, AZ

Join our West Phoenix team as a full-time Family Medicine Nurse Practitioner providing flexible, patient-centered primary care across multiple clinical settings. You’ll be a key member of a collaborative healthcare team, performing assessments, managing acute and chronic conditions, coordinating care with colleagues, and educating patients and families to promote health and continuity of care.

What you’ll do:

• Deliver comprehensive primary care to patients of all ages across varied clinical environments.
• Perform history and physical exams, order and interpret tests, and develop evidence-based treatment plans.
• Float between clinic sites as needed, adapting quickly to different workflows and patient populations.
• Communicate clearly with patients, families, and interdisciplinary team members to ensure coordinated care.
• Provide patient education on disease prevention, medication management, and self-care strategies.

What we’re looking for:

• Master’s or Doctoral degree in Nursing with a Family Nurse Practitioner focus.
• Active Family Nurse Practitioner certification.
• Arizona RN/APRN licensure and current DEA registration.
• Experience in family medicine or primary care settings.
• Strong interpersonal and communication skills and the ability to adapt to changing clinical environments.
