Internal or Family Medicine Physician Full-Time Primary Care (West Phoenix, AZ)

Full-time position in West Phoenix, AZ providing comprehensive primary care to individuals and families across the lifespan. Focus on preventive care, health maintenance, and management of acute and chronic medical conditions within an integrated care team.

Responsibilities

• Deliver routine primary care and urgent visits.
• Provide preventive services: screenings, immunizations, wellness exams.
• Diagnose and manage chronic diseases (diabetes, hypertension, COPD, heart disease, etc.).
• Develop and document treatment plans; coordinate referrals and follow-up.
• Educate patients on disease prevention, medication adherence, and lifestyle modification.
• Collaborate with care team to optimize quality metrics and patient outcomes.

Qualifications

• MD or DO degree.
• Board certified in Internal Medicine.
• Active Arizona medical license and DEA registration.
• Strong clinical, diagnostic, and communication skills.
