Job Description

Make a meaningful difference caring for adults with complex medical and social needs as a full-time primary care Nurse Practitioner in Memphis, Tennessee. Join a small, collaborative clinic that provides relationship-based primary care to people living with chronic illness, disabilities, behavioral health conditions, and significant social barriers to care.

You will practice whole-person medicine in an interdisciplinary setting alongside behavioral health clinicians, community health workers, and care coordinators. Using a trauma-informed, patient-centered approach, you’ll support patients in navigating complex healthcare systems, improving access, outcomes, and long-term well-being while building lasting therapeutic relationships.

Qualifications

  • Graduate of an accredited Nurse Practitioner program
  • Current, unrestricted NP license and active DEA registration with prescriptive authority
  • Board certification as FNP, AGPCNP, or equivalent
  • Primary care experience required; experience with Medicaid, underserved, behavioral health, or medically complex populations strongly preferred
  • Strong clinical assessment, diagnostic, and care-management skills
  • Commitment to compassionate, trauma-informed, patient-centered care
