Mobile Nurse Practitioner (Adult/Gerontology) — San Gabriel, CA

Join a full-time mobile Nurse Practitioner team delivering relationship-centered care across San Gabriel Valley and East Los Angeles. You’ll provide high-quality primary care through home visits, community outreach, and virtual appointments within a patient-focused, value-based organization that emphasizes prevention, proactive interventions, and coordinated care to improve long-term health outcomes.

This role offers substantial clinical autonomy while collaborating with an interdisciplinary team committed to quality over volume—focused on preventing illness, managing chronic conditions, and supporting patients in their own environments.

Position highlights

• Schedule: Full-time, Monday–Friday, 8:00 AM–5:00 PM
• Base salary: $130,159–$195,238
• $10,000 sign-on bonus
• Mobile practice: home visits, community-based care, and telehealth
• Mileage reimbursement
• Quarterly incentive plan: up to 10% of quarterly earnings

Qualifications

• Active, unrestricted California Nurse Practitioner license
• Active DEA registration
• Current BLS certification
• Strong clinical assessment, diagnostic, and decision-making skills
• Able to practice independently and collaborate effectively with a care team
• Commitment to patient-centered, preventive, value-based care
• Excellent communication and relationship-building abilities
• Comfortable delivering care in home, community, and virtual settings
