Job Description
Join a growing, patient-centered team delivering hands-on primary care to medically complex Medicare members in skilled nursing facilities, long-term care, and other institutional settings across the Las Vegas area. In this full-time role you’ll provide comprehensive, preventive, and chronic disease management for seniors enrolled in an Institutional Special Needs Plan (ISNP), working closely with interdisciplinary teams to improve outcomes and reduce avoidable hospitalizations.
What you’ll do
- Deliver primary care and ongoing management for medically complex residents in SNFs, LTCs, and post-acute settings.
- Lead proactive care plans focused on preventing complications and avoiding unnecessary hospital transfers.
- Collaborate daily with nurses, social workers, therapists, and facility staff to coordinate care.
- Document thoroughly for quality metrics (HCC, HEDIS, STARs) and support accurate coding.
- Travel between facilities around the Las Vegas area as needed.

- Base salary: $211,369–$317,053

What we’re looking for
- Master’s or Doctor of Nursing Practice (DNP) with FNP, AGNP, or equivalent nurse practitioner specialty.
- Current Nevada NP license (or ability to obtain) and DEA registration; prescriptive authority/collaboration agreement as required by state law.
- National NP certification and active clinical practice privileges.
- Comfort and experience practicing in institutional/post-acute care settings; 2+ years in SNF, LTC, post-acute, or complex senior care preferred.
- Familiarity with HCC coding, Medicare Advantage, HEDIS, and STARs documentation is a plus.
- Strong interpersonal skills, clinical judgment, and the ability to work independently while partnering with facility teams.
- Willingness to travel between healthcare facilities within Nevada.
