Job description:
Renuven Health Partners is a physician-led medical group dedicated to delivering extraordinary, patient-centered care for older adults in post-acute and long-term care settings. Our mission is to help patients regain strength and return home safely, while avoiding unnecessary complications and hospital readmissions.
We are seeking a Nurse Practitioner or Physician Assistant to join our team in caring for patients in skilled nursing and long-term care facilities in Anderson, SC.
This is a full-time position with a 7 on / 7 off, 12-hour shift schedule.
Call coverage is provided by a national telehealth group, with the NP/PA available for back-up and oversight of any hospital transfers.
Experience in post-acute or long-term care (PA/LTC) is preferred, but providers with other relevant experience are encouraged to apply.
Key Responsibilities
- Serve as the primary care provider for patients in skilled nursing and long-term care settings, ensuring high-quality, patient-centered care.
- Perform comprehensive patient assessments and evaluations to develop individualized treatment plans aligned with each patient’s goals.
- Diagnose and manage acute and chronic conditions, coordinating timely interventions to promote recovery and prevent unnecessary hospitalizations.
- Accurately document all patient encounters and care plans in medical records, including appropriate ICD-10 coding, to support continuity of care and compliance.
- Partner closely with nursing staff, facility teams, and other healthcare professionals to deliver coordinated, compassionate care.
- Educate patients and families about health conditions, treatment options, and preventive care, fostering understanding and shared decision-making.
- Complete required Medicare recertification visits and documentation to ensure uninterrupted patient services and regulatory compliance.
Qualifications
- Master’s or Doctoral degree in Nursing (MSN or DNP) or Master's of Physician Assistant Studies (MPAS).
- National board certification (AANP, ANCC, or NCCPA) in applicable specialty.
- Current, active state license.
- Current DEA registration (or eligibility to obtain).
