Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing (Traverse City, MI)

We’re expanding our team that delivers patient-centered, high-quality care to residents of skilled nursing facilities around Traverse City, Michigan. If you’re a compassionate Nurse Practitioner or Physician Assistant who enjoys working with older adults in a collaborative, team-based setting, we’d love to hear from you.

What the role looks like

• Flexible scheduling with full-time and PRN opportunities.
• Full-time shifts are six hours; rounds typically start between 7:00 a.m. and 10:00 a.m.
• No call and no overnight coverage.
• You’ll perform daily rounds, manage both acute and chronic conditions, prescribe medications and treatment plans, and coordinate care with facility staff and families.
• Documentation is streamlined: we provide an EHR with AI Scribe and voice dictation, plus a company iPad with LTE.
• Clinical decision support is available through UpToDate®.

What we’re looking for

• Active, unrestricted Nurse Practitioner or Physician Assistant license.
• Board certification (examples: ANCC, AANPCB, PNCB, AACN, or NCCPA).
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred.
