Clinical Lead Adult Nurse Practitioner - TeamCare Leader, Bronx; $162-190K, M-F

Join our TeamCare program in The Bronx as the Clinical Lead NP assigned to a TeamCare site. This role delivers comprehensive adult primary care, leads clinical operations at the site, and coordinates care. In the Medical Director’s absence or per their direction, you will represent and provide clinical leadership for the location.

Key responsibilities

• Provide direct primary care to adult patients onsite.
• Serve as clinical leader for the assigned TeamCare site; support implementation of clinical policies and workflows.
• Coordinate care across multidisciplinary teams and community resources.
• Act as clinical liaison to the Medical Director when delegated.
• Participate in quality improvement, case review, and clinical documentation oversight.
• Mentor and support clinical staff as needed.

Schedule

• Monday-Friday, 8:30 AM-5:30 PM (1-hour lunch); no routine evenings or weekends.

Compensation & benefits

• Base salary: $162,000-$190,000 (dependent on experience) plus bonus.

Minimum qualifications

• Graduate of an accredited Nurse Practitioner program; eligible for NYSED recognition.
• Active, unrestricted NY NP and RN licenses.
• AANP or ANCC board certification and current DEA registration.
• Current BLS certification.
• Active CAQH profile; current Medicare and Medicaid provider IDs.
• 2+ years clinical experience in primary care, home care, care management, or related settings; 1+ year caring for adult patients preferred.
• Strong clinical judgment, communication, analytical, and organizational skills.
• Proficiency with Microsoft Office and electronic clinical systems.
• Bilingual skills a plus.
