Board-Certified Internal Medicine Site Physician - Bronx, NY (Mon-Fri, Geriatrics Focus)

Join our team in the Bronx and help provide compassionate, comprehensive care to a largely frail and elderly patient population at a Community Health/Day Treatment Center. This full-time role follows a stable Monday-Friday schedule (8:00 AM-5:00 PM) and combines site-based care with occasional home visits. You’ll supervise clinical and administrative aspects of patient care and collaborate closely with an interdisciplinary team to deliver high-quality, patient-centered services.

Key responsibilities

• Oversee professional, clinical, organizational, and administrative aspects of patient care at the center
• Coordinate treatment plans with social workers, therapists, nurses, and other team members
• Provide direct medical care to a frail, elderly population both on-site and through home visits
• Ensure high standards of quality, safety, and patient-centered practice

Qualifications

• Graduate of an accredited medical school
• Completion of an Internal Medicine or Family Medicine residency
• Board certified in Internal Medicine or Family Medicine (required)
• Geriatric medicine certification, fellowship, or relevant experience preferred
• Minimum one year of experience caring for frail or elderly patients preferred
• Home-care experience preferred; must be willing to provide home-care services
• Current, unrestricted New York State medical license and active DEA
• Active Medicare and Medicaid provider IDs; current CAQH profile
• Excellent clinical judgment, communication, organizational, and interpersonal skills
• No malpractice or loss-of-license history related to practice at the DTC
• Bilingual skills are a plus
