Site Physician Assistant (Adult/Geriatrics) - Brooklyn, NY

We are seeking an experienced Physician Assistant to join our Brooklyn team as a full-time Site PA providing high-quality medical care at our Diagnostic Treatment Centers and through home visits. This role is ideal for a PA who enjoys caring for frail and elderly patients and delivering comprehensive, patient-centered care across clinical and home settings.

Schedule

• Monday-Friday, 8:30 AM-5:30 PM (1-hour lunch)

Compensation & benefits

• Base salary $158,000-$178,000 plus bonus

Key responsibilities

• Provide professional, clinical, organizational, and administrative medical care for elderly and frail patients
• Deliver geriatric-focused assessments, treatment planning, and follow-up in clinic and home settings
• Coordinate care with interdisciplinary teams, families, and community resources
• Maintain accurate medical records and comply with regulatory and billing requirements

Qualifications

• Graduate of an accredited Physician Assistant program
• NCCPA certification required
• Active, unrestricted NY State PA license and NY DEA registration
• Current BLS certification
• Active CAQH profile and Medicare/Medicaid provider IDs
• Minimum one year experience with frail or elderly populations preferred
• Geriatric medicine diploma, fellowship, or equivalent clinical experience preferred
• Experience in geriatrics, palliative care, and home-care services preferred; must be willing to provide home visits
• Strong verbal, written, clinical, and interpersonal communication skills
• No malpractice history or license restrictions related to DTC practice
• Bilingual skills a plus
