Board-Certified Internal Medicine Physician - Monday-Friday Brooklyn, Geriatrics-Focused, $240-280K

We are seeking a Board-Certified physician to provide comprehensive medical care at a Community Health/Day Treatment Center in Brooklyn, NY. This full-time role focuses on the frail and elderly, offering a stable Monday-Friday schedule and the chance to lead clinical services within an interdisciplinary team.

Key responsibilities

• Oversee professional, clinical, organizational, and administrative aspects of patient care.
• Provide site-based and home-care services as needed.
• Coordinate with nurses, social workers, therapists, and other team members to deliver high-quality, patient-centered care.
• Ensure documentation, compliance, and continuity of care for a complex geriatric population.

Compensation and benefits

• Base salary $240,000-$280,000 (depending on experience) plus bonus.
• Full benefits package.
• Regular schedule: Monday-Friday, 8:00 AM-5:00 PM.
• Collaborative, interdisciplinary practice environment.

Required qualifications

• Graduate of an accredited medical school.
• Completion of an Internal Medicine or Family Medicine residency.
• Board certification in Internal Medicine or Family Medicine.
• Current, unrestricted New York State medical license and active DEA license.
• Active Medicare and Medicaid provider IDs and current CAQH profile.
• Excellent clinical judgment, communication, organizational, and interpersonal skills.
• No history of malpractice or loss-of-license related to practice at the Day Treatment Center.

Preferred qualifications

• Geriatric medicine certification, fellowship, or experience.
• Minimum one year caring for frail or elderly patients.
• Home-care experience and willingness to provide home visits.
• Bilingual skills.
