Board-Certified Adult Site Physician - Monday-Friday, Geriatric Care in Manhattan (NYC)

Join a collaborative team in Manhattan delivering compassionate care at a Community Health/Day Treatment Center. In this full-time, Monday-Friday role (8:00 AM-5:00 PM) you’ll provide comprehensive medical care to a predominantly frail and elderly patient population, splitting time between site-based services and home visits. You will lead the medical, clinical, organizational, and administrative aspects of patient care and work closely with an interdisciplinary team to coordinate high-quality, patient-centered services.

Compensation and benefits

• Base salary $240,000-$280,000 (depending on experience) plus bonus

What you’ll do

• Provide primary medical care for frail and elderly patients in the Day Treatment/Community Health setting
• Oversee clinical standards, documentation, and care coordination
• Support and collaborate with nurses, social workers, therapists, and other team members to deliver integrated, patient-centered care
• Participate in administrative and organizational activities that support high-quality service delivery

Qualifications

• Graduate of an accredited medical school
• Completion of Internal Medicine or Family Medicine residency
• Board certified in Internal Medicine or Family Medicine (required)
• Geriatric certification, fellowship, or relevant experience preferred
• At least one year caring for frail or elderly patients preferred
• Experience in home-care services preferred; willingness to provide home visits required
• Current, unrestricted New York State medical license
• Active DEA registration
• Current CAQH profile and active Medicare and Medicaid provider IDs
• Strong clinical judgment, communication, organizational, and interpersonal skills
• No malpractice or loss-of-license history related to DTC practice
• Bilingual skills a plus
