Post-Acute Care Nurse Practitioner or PA - FT Mon-Fri, No Weekends/Call, $120K+ (Bennington, VT)

Join our expanding Post-Acute Care team in Bennington, VT, delivering compassionate, patient-centered care to individuals transitioning from acute hospital settings to skilled nursing and rehabilitation facilities. This full-time, facility-based role focuses on managing chronic and acute conditions, supporting recovery, reducing readmissions, and optimizing patient outcomes through collaborative, interdisciplinary care.

Additional openings are available in Wakefield, RI and North Conway, NH.

• Schedule: Full-time, Monday-Friday, 9:00 AM-5:00 PM. No weekends or on-call responsibilities.

Compensation & benefits

• Competitive base salary (starting at $120,000) with total earnings typically $180,000-$220,000+.

Qualifications

• Graduate of an accredited NP or PA program.
• Active, unrestricted NP or PA license in the applicable state, or eligibility for licensure.
• National board certification and DEA registration (or eligibility).
• Minimum one year clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred.
• Strong clinical assessment, diagnostic, care-coordination, and EMR skills.
• Excellent communication, interpersonal skills, and ability to work independently while collaborating within multidisciplinary teams.
