Bilingual Primary Care Nurse Practitioner (English/Spanish) - Chula Vista, CA

Join a collaborative, patient-centered primary care team in Chula Vista, California focused on whole-person wellness, prevention, and long-term outcomes. You’ll see 10-15 patients per day with extended visit times that allow meaningful connections and comprehensive care - without the pressure of high-volume medicine.

Key responsibilities and team features

• Deliver family/primary care within a team-based model that includes an embedded Whole Health Coach
• Provide preventive care, chronic disease management, and population health-focused case management
• Participate in interdisciplinary collaboration and contribute to quality improvement and leadership initiatives
• Use electronic medical records and Google Workspace as part of daily workflows

Compensation & benefits

• Salary range: $119,313-$178,968 annually, plus annual incentive opportunities

Required qualifications

• NP board certification in family, primary care, or internal medicine
• Current, valid, unrestricted ARNP license (California) and valid DEA registration
• Bilingual proficiency in English and Spanish (essential)
• 3-5 years of primary care experience; minimum 2 years in case management emphasizing prevention and population health
• Team-based care experience and comfort working in EMR systems and Google Workspace
• Documentation of a negative TB test within 12 months prior to start date
