Mobile Nurse Practitioner (Adult/Geriatric) - Home & Community-Based, San Gabriel, CA

Join a full-time mobile practice delivering relationship-based, high-quality care across San Gabriel Valley and East Los Angeles. You will provide home visits, community-based care, and virtual appointments within a patient-centered, value-based organization that emphasizes preventive care, proactive interventions, and coordinated support to improve long-term outcomes. This role offers clinical autonomy while collaborating with an interdisciplinary team committed to better health for older adults.

Position highlights

• Schedule: Full-time, Monday-Friday, 8:00 AM-5:00 PM
• Competitive base salary: $130,159-$195,238
• $10,000 sign-on bonus
• Mobile practice: home visits, community-based care, and telehealth
• Mileage reimbursement
• Quarterly incentive plan: up to 10% of quarterly earnings

Qualifications

• Active, unrestricted California Nurse Practitioner license
• Active DEA registration
• Current BLS certification
• Strong clinical assessment and diagnostic skills
• Comfortable providing care in home, community, and virtual settings
• Ability to practice independently and collaborate effectively with an interdisciplinary team
• Commitment to patient-centered, preventive, and value-based care
• Excellent communication and relationship-building skills
