Mobile Adult/Geriatric Nurse Practitioner - Field-Based, Autonomy + Bonus (San Fernando, CA)

We’re looking for a compassionate Mobile Adult/Geriatric Nurse Practitioner to deliver acute, post-acute, and transitional care to medically complex and recently discharged patients across the San Fernando Valley and Antelope Valley. This field-based role combines clinical autonomy with collaborative teamwork and a focus on improving outcomes, preventing readmissions, and helping patients recover safely in their own homes.

What you’ll be doing

• Provide comprehensive assessments, diagnosis, and treatment for medically complex adults and older adults in the home setting
• Manage transitions of care for recently discharged patients, coordinating with hospitals, primary care, caregivers, and community resources
• Develop and implement individualized care plans that prioritize patient safety, comfort, and recovery
• Work independently in the field while partnering with an interdisciplinary team to ensure continuity and quality of care
• Document clinical encounters thoroughly and participate in quality improvement efforts to support our value-based, relationship-centered model

Compensation & benefits

• Base salary range: $119,313 - $178,968 annually

Qualifications

• Master of Science in Nursing with Adult/Geriatric Nurse Practitioner certification
• Active, unrestricted California RN and NP licenses
• Minimum of 1 year of clinical experience as a Nurse Practitioner
• Strong skills in assessment, diagnosis, and care management for medically complex patients
• Valid driver’s license and reliable transportation for field visits
• Ability to work independently while collaborating effectively with a multidisciplinary team
• Current TB test or willingness to obtain prior to employment
