Adult-Gerontology Nurse Practitioner (NP) - Mesa, AZ

Full-time position in Mesa, AZ providing high-quality primary and geriatric care to adult and older adult patients. The NP will perform comprehensive assessments, diagnose and manage acute and chronic conditions, develop treatment plans, prescribe medications, and coordinate care with interdisciplinary teams to optimize patient outcomes.

Responsibilities:

• Conduct comprehensive patient assessments and diagnostic evaluations.
• Provide primary care services including management of acute and chronic illnesses.
• Develop, implement, and modify individualized treatment plans; prescribe and manage medications within scope.
• Perform geriatric-focused assessments (cognitive, functional, medication reconciliation, falls risk) and implement appropriate interventions.
• Coordinate care with physicians, specialists, nurses, therapists, social workers, and caregivers.
• Deliver patient and family education on disease management, preventive care, medications, and advance care planning.
• Maintain clear, timely, and accurate medical records and documentation.
• Participate in community outreach, patient advocacy, and quality improvement activities as needed.

Requirements:

• Master’s or Doctoral degree in Nursing with Adult-Gerontology NP focus.
• Current Arizona NP licensure.
• Certification as an Adult-Gerontology Nurse Practitioner.
• Clinical experience in adult and geriatric healthcare.
• Strong clinical judgment, communication, and interdisciplinary collaboration skills.
