Part-Time Hospitalist Nurse Practitioner / Physician Assistant - Skilled Nursing (Mishawaka, IN)

Provide patient-centered primary care to residents in skilled nursing facilities across Mishawaka, Indiana, as part of a growing, collaborative healthcare team. This part-time role offers flexible scheduling, no on-call or overnight duties, and the chance to work closely with physicians and facility staff to deliver high-quality, evidence-based care.

Location opportunities: Mishawaka, IN. Additional openings nationwide in GA, IL, KS, MI, MO, OH, TN, and WI.

Key responsibilities

• Perform daily rounds and manage 15-20 patient visits per day on average.
• Assess and treat acute and chronic conditions, prescribe medications, and develop individualized treatment plans.
• Collaborate with nursing staff and interdisciplinary teams to coordinate care.
• Maintain compliance with clinical and regulatory standards and document encounters thoroughly.

Schedule & work environment

• Part-time with flexible scheduling; six-hour facility shifts.
• Rounds typically begin between 7:00 a.m. and 10:00 a.m.
• No call or overnight shifts.

Qualifications

• Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license.
• NP board certification by ANCC, AANPCB, PNCB, or AACN; PA certification by NCCPA.
• Board certified or board eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine preferred.
• Strong clinical judgment, excellent communication, and the ability to work independently while collaborating within a multidisciplinary team.
