Family Medicine Nurse Practitioner - Value-Based Primary Care (Tacoma, WA)

Join a collaborative care team delivering evidence-based primary care in Tacoma. You will provide comprehensive, patient-centered services within a medical home model, partnering with patients and an interdisciplinary team (including an onsite Whole Health Coach) to develop biopsychosocial care plans and lead clinical initiatives.

Key responsibilities

• Deliver evidence-based primary care to approximately 10-15 patients per day, emphasizing prevention, practical treatment of acute and chronic conditions, and disease/case management.
• Conduct comprehensive health histories, appropriate physical exams, diagnostic testing, and medication management, with access to onsite lab and pharmacy.
• Collaborate with patients and team members to create individualized care plans that respect diverse backgrounds and needs.
• Identify population health needs and promote participation in annual whole health evaluations, health coaching, and other health-promotion activities.
• Provide clinical leadership through active participation in team meetings, proactive communication, prioritization of initiatives, process improvement, and support for change management.
• Participate in a regional on-call/rotation schedule to cover after-hours care.

Compensation & benefits

• Base salary: $150,000

Qualifications

• NP or PA board certification with a background in primary care, family medicine, or internal medicine
• Current, unrestricted ARNP or PA license and valid DEA registration
• 3-5 years of primary care experience
• At least 2 years of case management experience with emphasis on health prevention and population health management
• Experience with electronic medical records (NextGen preferred)
• Proficiency in coding and documentation, including Medicare Advantage and HCC
