**Location: Windsor, CT**

Position Highlights
• Join a collaborative and well-established Family Medicine Primary Care team in Windsor, CT.
• Open to Family Nurse Practitioners (FNPs) and Physician Assistants (PAs).
• 100% outpatient Family Medicine/Primary Care setting serving patients from birth through geriatrics.
• Full-time position with a weekday-only schedule.
• No call required.
• Supportive practice consisting of 4 physicians and 1 APP.
• Embedded behavioral health provider and clinical pharmacist.
• Strong collaboration among providers, nurses, medical assistants, and front-office staff.

APP Role & Responsibilities
• Perform routine physical examinations and comprehensive patient assessments.
• Develop medical, educational, and psychosocial treatment plans for patients.
• Collaborate with physicians and other members of the care team to address patient health needs.
• Develop plans for further evaluation, including referrals to specialists and other healthcare professionals when appropriate.
• Provide treatment for conditions within the provider’s scope of practice and established protocols.
• Identify conditions outside the APP’s scope of practice and appropriately refer to a physician.
• Practice under established guidelines and in collaboration with and/or supervision of the practice physicians.
Practice & System Support
• Dedicated support and advocacy for Nurse Practitioners and Physician Assistants through an Office of Advanced Practice.
• Access to a large, integrated healthcare system, providing opportunities for continuity of care across Connecticut.
• Utilize EPIC and advanced clinical and informatics technology to support patient outcomes and streamlined workflows.
• Established mentorship and orientation programs for newly hired providers and those new to practice.

Qualifications
• Education/Training: Family Nurse Practitioner (FNP) or Physician Assistant (PA).
• Licensure: Unrestricted Connecticut medical/APRN license or eligibility.
• Experience: 1+ years of experience in primary care or a relevant outpatient setting.
