• Split role: approximately 50% direct patient care and 50% clinical leadership and administration
• Care delivered via telemedicine and home visits within a value-based care model
• Monday - Friday schedule, 8:30 am - 5 pm
• Full-time or part-time options available; minimum commitment of 3 days per week
• Serve as collaborating physician and first point of clinical escalation for Nurse Practitioners
• Focus on symptom management, goals-of-care discussions, and advance care planning
• Board-certified with experience in palliative care or leadership preferred
• Active Michigan license and DEA required
