Job Description
Provide evidence-based primary care to patients with cancer in both office-based and virtual settings in Virginia Beach, VA. You will work as part of an interdisciplinary oncology team to coordinate comprehensive care, manage chronic and complex conditions, support transitions of care, and help improve patient outcomes within a value-based care model.
Key responsibilities
- Deliver direct, evidence-based primary care to oncology patients in office and telehealth settings.
- Collaborate closely with oncology clinicians to align primary care with cancer treatment plans.
- Manage chronic and complex medical conditions, including congestive heart failure, frailty, and behavioral health concerns.
- Coordinate referrals and ongoing care with community primary care and specialty providers.
- Conduct transitions-of-care visits to ensure continuity and safety after hospitalizations or treatment changes.
- Participate in team huddles, case reviews, and care coordination activities.
- Escalate clinical concerns to the Market Medical Director as appropriate.
- Partner with clinical and practice leadership to streamline operations and enhance patient care.
- Support quality initiatives and work to achieve key clinical performance measures.
- Perform other duties as needed to support the practice and patient population.
Qualifications
- Current licensure and certification as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Minimum of 4 years of direct patient care experience in primary care, oncology, or a related setting.
- Demonstrated expertise in chronic disease management and/or palliative care.
- Experience caring for oncology or complex/high-acuity patients preferred.
- Strong clinical judgment, communication, care coordination, and teamwork skills.
- Comfortable working in a growing value-based care environment.
