Job Description
We’re expanding a team that delivers high-quality, patient-centered care to residents in skilled nursing facilities across Ossipee, New Hampshire. If you’re a compassionate Nurse Practitioner or Physician Assistant who enjoys working with older adults in a collaborative, team-based setting, we’d love to hear from you.
What you’ll do
- Provide daily rounds in skilled nursing facilities, managing acute and chronic conditions, prescribing and adjusting medications, and developing treatment plans.
- Work efficiently with a streamlined EHR (AI Scribe enabled) and voice dictation to document care.
- Use company-provided tools - including an iPad with LTE - and clinical resources like UpToDate® for point-of-care guidance.
- Coordinate with interdisciplinary teams to support residents’ overall health and quality of life.
Schedule and work environment
- Full-time positions with flexible scheduling.
- Typical full-time shifts are six hours; rounds usually start between 7:00 a.m. and 10:00 a.m.
- No overnight or on-call responsibilities.

What we’re looking for
- Active, unrestricted Nurse Practitioner or Physician Assistant license.
- Board certification (ANCC, AANPCB, PNCB, AACN, NCCPA, or equivalent).
- Experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred.
