Requirements:

• MD or DO from an accredited medical school
• Completion of an accredited U.S. Family Medicine residency
• Board Certified or Board Eligible in Family Medicine
• Valid Arizona medical license and DEA registration
• Current Arizona driver’s license
• Level One Fingerprint Clearance Card (Arizona DPS) for specialty behavioral health assignments
• Current CPR and AED certification for healthcare professionals (American Heart Association or American Red Cross standards, including cognitive and skills evaluations)
• Demonstrated clinical competence and familiarity with clinical methods and electronic medical records
• Strong interpersonal and communication skills; ability to provide respectful, culturally responsive care to diverse populations
• Basic proficiency in business English, spelling, punctuation, and computer use
