• Monday - Friday schedule with round-and-go model; finish when patients are seen
• Post-acute setting with lower-acuity patients in skilled nursing facilities
• Full-time, hospital-employed position with local physician-led leadership
• Internal Medicine or Family Medicine physicians encouraged to apply
• Manage medically complex post-acute, rehabilitation, and geriatric patients
