Hospitalist NP/PA - Full-Time SNF Care in Evansville, IN • Flexible 6-hr Shifts, No Nights/Call

Provide patient-centered primary care to residents in skilled nursing facilities across Evansville, IN. Work full time with a flexible daytime schedule; no call or overnight shifts.

Key responsibilities

• Perform daily rounds and manage 15-20 patient visits per day on average
• Assess and manage acute and chronic conditions; develop and implement treatment plans
• Prescribe medications and order/interpret tests as needed
• Coordinate care with facility nursing staff and interdisciplinary teams
• Maintain compliance with clinical and regulatory standards
• Document care in company systems (company-provided iPad with LTE)

Schedule

• Full-time
• Six-hour facility shifts; rounds typically start between 7:00 a.m. and 10:00 a.m.
• No on-call or overnight responsibilities

Minimum qualifications

• Active, unrestricted U.S. license as a Nurse Practitioner or Physician Assistant
• NP board certification (ANCC, AANPCB, PNCB, or AACN) or PA certification (NCCPA)
• Board certified or board eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
• Strong clinical judgment, communication skills, and ability to work independently and collaboratively within a multidisciplinary team
