Attending Physician - Hospitalist for Skilled Nursing (Flexible Schedule, No Call, Collaborative Team)

Seeking a full-time Attending Physician to provide patient-centered care to residents in skilled nursing facilities across the Lima, Ohio area. This role offers clinical autonomy within a collaborative team-based model, oversight of medical care for assigned residents, and participation in quality initiatives. No on-call or overnight duties.

Responsibilities

• Serve as Attending Physician for residents in assigned skilled nursing facilities
• Provide diagnosis, treatment, and ongoing management of acute and chronic conditions
• Develop individualized care plans and coordinate transitions of care
• Supervise and collaborate with Nurse Practitioners and Physician Assistants
• Participate in Chronic Care Management, Transitional Care Management, Remote Patient Monitoring, and other quality improvement programs
• Use EHR, voice dictation, AI-powered scribe tools, and point-of-care resources (UpToDate) for documentation and decision support

Requirements

• MD or DO with active, unrestricted U.S. medical license or eligibility for licensure
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
• Strong communication, leadership, and clinical decision-making skills
