Full-time Family Medicine position in South Bend, Indiana, with additional opportunities in Elkhart and Michigan City.

Practice overview

• Provide comprehensive primary care to patients of all ages in a patient-centered, multidisciplinary setting.
• Work with an experienced team focused on coordinated, evidence-based care and long-term patient relationships.

Key responsibilities

• Diagnose and manage acute and chronic conditions.
• Deliver preventive care, wellness exams, and health screenings.
• Coordinate care with specialists and interdisciplinary team members.
• Maintain accurate, timely documentation in the electronic medical record.
• Participate in quality improvement and population health initiatives as needed.

Requirements

• MD or DO from an accredited medical school
• Board Certified or Board Eligible in Family Medicine
• Completed accredited Family Medicine residency
• Eligible for or holding an unrestricted Indiana medical license
• DEA certificate or eligibility to obtain
• Strong clinical, communication, and interpersonal skills
