Hospitalist Nurse Practitioner / Physician Assistant - Skilled Nursing (Fort Worth, TX)

Join a growing team delivering patient-centered care to residents in skilled nursing facilities across Fort Worth, Texas. We seek compassionate Nurse Practitioners and Physician Assistants dedicated to improving the health and quality of life of older adults within a collaborative, team-based setting.

What you’ll do

• Perform daily rounds in skilled nursing facilities, assessing and managing acute and chronic conditions.
• Develop and prescribe treatment plans and medications.
• Coordinate care with interdisciplinary teams, families, and facility staff.
• Document efficiently using a streamlined EHR with AI Scribe and voice dictation on a company-provided iPad with LTE.
• Use UpToDate® for point-of-care clinical guidance.

Schedule and work environment

• Full-time positions with flexible scheduling.
• Six-hour facility shifts; rounds typically start between 7:00 a.m. and 10:00 a.m.
• No on-call or overnight responsibilities.

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license.
• Board certification (ANCC, AANPCB, PNCB, AACN, NCCPA, or equivalent).
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred.
