Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing (Lima, OH)

Come join a growing team dedicated to delivering compassionate, patient-centered care to residents in skilled nursing facilities across Lima, Ohio. We’re looking for Nurse Practitioners and Physician Assistants who enjoy working with older adults and thrive in a collaborative, team-based setting.

What the role looks like

• Full-time positions with flexible scheduling.
• Six-hour facility shifts for full-time providers; rounds generally start between 7:00 a.m. and 10:00 a.m.
• No overnight shifts or on-call obligations.
• Perform daily rounds, manage acute and chronic conditions, and develop and prescribe treatment plans.
• Document efficiently using a streamlined EHR with AI Scribe, voice dictation, and a company-provided iPad with LTE.
• Access UpToDate® for real-time clinical guidance.
• New graduates are encouraged to apply.

What we’re looking for

• Active, unrestricted Nurse Practitioner or Physician Assistant license.
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA).
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred but not required.
