Hospitalist Nurse Practitioner / Physician Assistant - SNF (Henry, IL)

Join a team delivering compassionate, patient-centered care across post-acute and skilled nursing facilities in Henry, Illinois. We’re looking for a board-certified Nurse Practitioner or Physician Assistant who values clinical autonomy, flexible scheduling, and working with a supportive, collaborative team.

What you’ll do

• Provide comprehensive assessments, develop and manage care plans, and coordinate with facility staff to ensure high-quality patient outcomes.
• Work six-hour shifts, with rounds typically beginning between 7:00 and 10:00 AM.
• Focus on in-person clinical care - there are no overnight or on-call requirements.
• Use a company-provided iPad with LTE for efficient charting and communication.
• Operate with independence while having access to a skilled clinical team for consultation and support.

Who we’re looking for

• Active NP or PA license to practice in Illinois.
• Current national certification: ANCC, AANPCB, PNCB, or AACN for NPs, or NCCPA for PAs.
• Clinical experience in internal medicine, family medicine, emergency medicine, general practice, physiatry, geriatrics, or a related field.
• Strong clinical assessment, decision-making, communication, and organizational skills.
• Ability to practice autonomously within scope while collaborating effectively with facility staff.

Compensation & benefits

• Competitive base pay range: $104,000-$169,000, with the potential to exceed industry benchmarks.
