Nurse Practitioner | Primary Care Outpatient | Bronx, NY
About the Opportunity
- Full-time, permanent outpatient primary care NP role, 40 hours/week
- Value-based care model keeping older adults on Medicare healthy and out of the hospital
- See patients independently, with physician collaboration as needed
- Build long-term relationships with patients through longitudinal care
- Focus on in-clinic primary care, care coordination and population health; specialists and hospitals handle specialty and acute care

- Care team: physician, MAs, RNs and care managers
- Outpatient only
Requirements
- MSN, DNP or NP Certificate Program graduate
- 2 years of primary care experience
- Active, non-probationary NP license
- Active DEA
- Fluent Spanish required (no flexibility)
Preferred
- Knowledge of HCC documentation, ICD-10 coding and Health Risk Assessments (HRAs)
