We are seeking a full-time primary care physician.

* Your role will include patient care rounding in a post-acute care setting in a skilled nursing facility located in the Utica, NY, area.
* You will round 5 days a week with the flexibility to choose your rounding schedule.
* Day call only on days covering
* Day call weekend rotation

**Overview**

* Primary care rounding in skilled nursing facilities
* Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
* Collaborate with clinicians, facility leaders and others regarding care and services
* Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
* Provide oversight, guidance, and mentorship to our nurse practitioners
* Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care
* Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issues

**Qualifications**

* Minimum of 1 year primary care experience
* Post-acute care experience a plus, but not require
* Hospitalist, internal medicine, family medicine, emergency medicine and related specialties encouraged to apply.
* Active and unrestricted New York license or application in process
* Active and unrestricted DEA/prescriptive authority or application in process

**Compensation**

* Expected compensation is $183,222 to $221,114 annually with no cap on productivity income potential.
* Uncapped earning potential
* Paid professional liability insurance with tail coverage
