**Geriatric Medical Director Opening – PACE Program (West Springfield, Massachusetts)**
A dedicated full-time Medical Director is needed to lead a Program of All-Inclusive Care for the Elderly (PACE) in West Springfield, Massachusetts. This leadership role provides comprehensive oversight of participant medical care and care coordination for seniors with chronic conditions, helping them remain independent and safely supported in their community.
**About the Opportunity**
The PACE model emphasizes that seniors with chronic care needs thrive best when cared for within their community. Participants are age 55 and older, certified to need nursing home level care, and able to live safely in the community at enrollment. Only about 7% of PACE participants nationally reside in a nursing home — if skilled care is needed, the program funds and coordinates it.
This role oversees the medical care model, supports independence, and ensures delivery of the full continuum of care and support services.
**Position Highlights**
-Provide general medical direction and supervision of participant medical care
-Oversee clinical outcomes and the Quality Assurance Performance Improvement (QAPI) program
-Develop and maintain medical policies, procedures, standards, and clinical protocols
-Provide direction and supervision to Primary Care Providers (PCPs) and Advanced Practice Providers (APPs)
-Work collaboratively with leadership and the interdisciplinary care team
-Administrative oversight from the Executive Director
**Responsibilities**
-Conduct history and physical exams on new participants and perform periodic reassessments
-Develop medical treatment plans and provide medically necessary services in various care settings
-Interview and evaluate applicants and caregivers; participate in intake and assessment decisions
-Discuss prognosis, treatment options, and advance care planning with participants and caregivers
-Collaborate with nursing team on participant treatment plans
-Serve as part of the leadership team to achieve operational goals
-Participate in interdisciplinary team meetings, case conferences, and family decision-making
-Educate participants and families about chronic disease management and preventative care
-Perform physical exams, order and evaluate tests, and document results systematically
-Participate in provider on-call rotation as required
