Requirements:

  • Current Physical Therapy license in practicing state(s) or proof of eligibility.
  • Master’s degree in Physical Therapy; or Master’s in a related field plus a Bachelor’s in Physical Therapy; or Bachelor’s in Physical Therapy with ≥5 years PT experience.
  • Valid driver’s license or reliable transportation.

Key Responsibilities:

  • Screen, examine, evaluate, diagnose impairments, set goals, and develop individualized treatment plans; document all services and progress.
  • Provide therapeutic interventions including therapeutic exercise (aerobic, balance, coordination, strength, flexibility), functional training, manual therapy, airway clearance, integumentary repair, electrotherapeutic/physical agent/mechanical modalities, and chronic disease management.
  • Perform mobility training, instruct patients/families, assess progress and behavior, and conduct routine reexaminations to modify or advance plans of care.
  • Conduct home and community mobility assessments and provide safety recommendations and fall-risk interventions.
  • Incorporate health, wellness, and preventive programming into care.
  • Collaborate with the rehabilitation team and refer patients to other disciplines/agencies as needed.
  • Supervise PT assistants, temporary licensees, students, and support staff in accordance with state licensure and professional standards.
  • Maintain timely clinical and billing documentation; adhere to department schedules and attend required meetings and patient/staff conferences.
  • Order, maintain, clean, and report repair needs for supplies and equipment.
  • Promote and maintain a safe environment; identify and eliminate fire and safety hazards.
  • Ensure patients and families receive high-quality, compassionate care.
  • Perform other duties as assigned by management.
