Schedule:

• Days, 8 hours/day
• Typical shift: 9:00 AM – 5:30 PM
• Rotating weekends: approximately 4 weekends per year

Key Responsibilities:

• Provide outpatient physical therapy evaluation, treatment, and documentation in accordance with facility policies and standards of care.
• Develop and implement individualized treatment plans and progress goals.
• Communicate with patients, families, and interdisciplinary team regarding care and discharge planning.
• Maintain accurate clinical records and comply with regulatory and billing requirements.

Qualifications:

• Licensed Physical Therapist (PT)
• Bachelor’s degree (minimum)
• Basic Life Support (BLS) certification required within one month of hire or transfer
