Home Health Physical Therapist (PT) — Days, Contract in Pittsfield, MA

Location: Pittsfield, MA

Assignment: Contract / Travel
Setting: Home Health (hospital-based referrals)
Shift: Days — 8:00 AM–4:30 PM (8 hours/day)
Schedule Notes: Rotating weekends and holiday coverage as needed
Pay: $2,952 per 40-hour week

Required Qualifications:

• Current Massachusetts PT license
• BLS (AHA)
• Valid driver’s license
• Minimum 2 years combined acute care (hospital-based) and home care experience
• Certified home health experience with proficiency in OASIS documentation
• Able to work independently with minimal supervision

Preferred:

• Additional acute care experience

Responsibilities:

• Provide skilled physical therapy services in the home-health setting
• Complete OASIS assessments and related documentation
• Coordinate care with interdisciplinary team and referring facilities
• Manage patient caseload autonomously, including evaluation, treatment planning, and discharge planning

Contract Details:

• Travel/contract assignment; start ASAP
• Weekend/holiday rotation required when needed
