Advanced practice provider (PA/NP) opportunity with an established senior health clinic

Full-time role providing comprehensive care to older adults in clinic and affiliated nursing home settings

Monday"Friday schedule with no weekend or holiday requirements, supporting strong work"life balance

Responsibilities include geriatric assessments, chronic disease management, coordination of care, and collaboration with a multidisciplinary team focused on quality of life for aging adults
