Contract Physical Therapist (PT) — Santa Clara, CA (13-Week Travel Assignment)

Join a dynamic healthcare team in Santa Clara, CA, on a 13-week contract/travel assignment beginning ASAP. This full-time role follows an 08:00 AM–04:30 PM schedule. Compensation: Travel rate $2,121/35 hrs; Local rate $2,090/35 hrs.

Required credentials

• Active Physical Therapy state license
• CPR and BLS certification from AHA or Red Cross (with AED)

Key responsibilities

• Perform thorough evaluations, review medical history, and collaborate with interdisciplinary teams to determine patient needs and goals.
• Develop and implement individualized treatment plans and targeted rehabilitation goals.
• Deliver therapeutic exercise programs to improve strength, flexibility, endurance, and coordination.
• Provide manual interventions (joint mobilization, soft tissue work, stretching) to restore mobility and reduce pain.
• Utilize modalities (ultrasound, electrical stimulation, heat/cold) as appropriate to manage pain and support tissue healing.
• Instruct patients on safe use of assistive devices (crutches, canes, walkers, wheelchairs) and design personalized home exercise programs.
• Educate patients and families on diagnoses, prognosis, body mechanics, ergonomics, and injury prevention.
• Monitor and document patient progress, adjusting treatment plans and communicating updates to physicians, nurses, and occupational therapists.
• Apply evidence-based pain management strategies and support patients in regaining independence after surgery, injury, or illness.
• Maintain accurate clinical records, adhere to legal/ethical standards, obtain informed consent, and demonstrate cultural sensitivity.
• Perform other duties as assigned by management.
