**Location: Sanford, Michigan**

• Full-time position with four 10-hour days, alternating Monday and Friday off
• Call: 1:11 with nurse triage line support
Location/Community:

Patient & Procedure Mix:
• Comprehensive family medicine care from birth through adulthood
• Preventive health, acute and chronic disease management
• Outpatient visits and in-office procedures
Requirements:
• U.S. medical degree or foreign equivalent
• Board-eligible or board-certified in Family Medicine
• Completion of Family Medicine residency
• Eligible for an unrestricted Michigan medical license and Controlled Substance License
Practice Details:
• Uses system-wide EPIC EMR
• PCMH-designated with top patient quality scores
• On-site Nurse Care Manager, Mental Health services, and Clinical Pharmacist
• Collaborative leadership and supportive staff environment
