Overview:
Join a collaborative rehabilitation team delivering high-quality, patient-centered physical therapy services. You will evaluate, plan, implement, and document individualized treatment programs while supervising support personnel and ensuring a safe, professional clinical environment.

Qualifications:

  • Valid license to practice Physical Therapy in the appropriate state(s), or proof of eligibility.
  • Master’s degree in Physical Therapy OR a Master’s in a related field plus a Bachelor’s in Physical Therapy OR a Bachelor’s in Physical Therapy with a minimum of 5 years’ PT experience.
  • Valid driver’s license or reliable transportation.

Key Responsibilities:

  • Evaluate, diagnose, and document patient conditions; develop measurable treatment goals and individualized care plans.
  • Deliver and progress evidence-based interventions, including therapeutic exercise (aerobic, balance, coordination, strength, flexibility), functional training, manual therapy, airway clearance, wound/integrity management, electrotherapeutic and mechanical modalities, and health/wellness and chronic disease management.
  • Conduct fall risk assessments, home and community mobility evaluations, and provide safety recommendations.
  • Instruct patients and caregivers in mobility, adaptive strategies, and self-management; monitor and document progress and behavioral responses.
  • Perform routine reexaminations and modify treatment plans as indicated.
  • Collaborate with interdisciplinary team members; refer to other disciplines and agencies as appropriate.
  • Supervise Physical Therapy Assistants, temporary licensees, students, and support staff in accordance with state licensure and professional standards.
  • Maintain timely, accurate clinical and billing documentation and participate in patient care and staffing conferences and required meetings.
  • Order, maintain, and report repairs for supplies and equipment; promote and maintain a safe environment by identifying and correcting hazards.
  • Integrate health and wellness programming into care plans.
  • Provide compassionate, high-quality service to patients and families.
  • Other duties as assigned by management.
