Minimum Qualifications

  • Valid Physical Therapy license in the practicing state(s) or proof of eligibility.
  • Education: Master’s degree in Physical Therapy; OR Master’s in a related field plus a Bachelor’s in Physical Therapy; OR Bachelor’s in Physical Therapy with 5 years PT experience.
  • Valid driver’s license or reliable transportation.

Key Responsibilities

  • Screen, examine, evaluate, diagnose, and document patient status, impairments, functional limitations, and treatment plans.
  • Establish treatment goals and implement interventions, including therapeutic exercise (aerobic, balance, coordination, strength, flexibility), functional training, manual therapy, airway clearance, integumentary repair, electrotherapeutic and mechanical modalities, health/wellness education, chronic disease management, fall-risk assessment/intervention, and case management.
  • Select and adapt activities to patients’ physical and cognitive capacities; instruct and evaluate mobility skills; perform routine reexaminations and modify/progress plans.
  • Conduct home and community mobility/safety assessments and provide recommendations.
  • Collaborate with the rehabilitation team to select appropriate therapeutic regimens and refer patients to other disciplines or agencies as needed.
  • Supervise PT assistants, temporary licensees, students, and support personnel in accordance with state licensure and professional standards.
  • Maintain timely clinical and billing documentation; adhere to department/patient schedules; attend work-related meetings and patient care/staffing conferences.
  • Order, maintain, clean, and report equipment needs and repairs; ensure availability of necessary supplies and equipment.
  • Promote and maintain a safe environment; identify and eliminate fire and safety hazards.
  • Ensure patients and families receive high-quality, compassionate care.
  • Incorporate health and wellness programming into treatment plans.
  • Perform other duties as assigned by management.
