Minimum Qualifications

  • Valid Physical Therapy license for practicing state(s) or proof of eligibility.
  • Education: Master’s degree in Physical Therapy; OR Master’s in a related field plus a Bachelor’s in Physical Therapy; OR Bachelor’s in Physical Therapy with 5 years PT experience.
  • Valid driver’s license or reliable transportation.

Core Responsibilities

  • Provide comprehensive PT services: screen, examine, evaluate, diagnose impairments, set goals, plan and document treatment (including therapeutic exercise—aerobic, balance, coordination, strength, flexibility—functional training, manual therapy, airway clearance, integumentary repair, electrotherapeutic/physical agent/mechanical modalities, health/wellness education, chronic disease management, fall risk assessment/intervention, and case management).
  • Select and adapt activities and programs to patients’ physical and cognitive capacities; instruct and assess mobility skills; perform routine reexaminations and modify/progress treatment plans.
  • Conduct home and community mobility assessments and provide safety recommendations.
  • Collaborate with the interdisciplinary rehabilitation team to determine appropriate therapeutic regimens and refer patients to other disciplines/agencies as needed.
  • Supervise PT Assistants, temporary licensees, students, and support personnel in accordance with state licensure and professional standards.
  • Ensure timely clinical and billing documentation, adhere to department and patient schedules, and attend required meetings and care/staffing conferences.
  • Order, maintain, clean, and report needed repairs for supplies and equipment.
  • Promote and maintain a safe environment; identify and eliminate fire and safety hazards.
  • Provide high-quality, compassionate care and ensure patients and families receive appropriate information and support.
  • Perform other duties as assigned by management.
