Outpatient Physical Therapist (Days) — Contract Travel in Springfield, VT — $2,600/40-hr, No Weekends

Assignment: Contract / Travel
Schedule: Days, 8-hour shifts, no weekends
Rate: $2,600 per 40 hours

Scope:
Provide outpatient physical therapy; must be able to treat the typical outpatient caseload. Flexibility required to cover inpatient PT as needed.

Required qualifications:

• Doctorate in Physical Therapy from an accredited program
• Minimum 2 years current PT experience
• Vermont PT license (or compact license valid in VT); New Hampshire license acceptable if applicable
• Active American Heart Association BLS
• Ability to work with PTAs
• Experience in rehab settings; previous travel assignment experience strongly preferred
• CPSI EMR experience highly preferred
• Traveler must have a permanent address and personal vehicle (no public transport in this small community)

Documentation (to be provided):

• Updated CAQH (after start)
• NPI number (after start)

Additional:

• Must be able to treat all common outpatient diagnoses
• Willingness to float to inpatient PT as required
