Requirements

• Valid Physical Therapy license in the practicing state or proof of eligibility.
• One of:
  • Master’s degree in Physical Therapy, or
  • Master’s in a related field plus a Bachelor’s in Physical Therapy, or
  • Bachelor’s in Physical Therapy plus 5 years’ PT experience.
• Valid driver’s license or reliable transportation.

Key Responsibilities

• Screen, examine, evaluate, diagnose, document and establish treatment plans/goals.
• Provide and document therapeutic interventions (therapeutic exercise, functional training, manual therapy, airway clearance, wound/integumentary care, electrotherapeutic/physical agent/mechanical modalities).
• Manage health/wellness education, chronic disease management, fall risk assessment/intervention, and case management.
• Select and adapt activities to patients’ physical and mental capacities; instruct in mobility skills; evaluate progress; perform routine reexaminations and modify treatment as needed.
• Conduct home and community mobility assessments and provide safety recommendations.
• Supervise PT assistants, temporary licensees, students, and support staff per state and professional standards.
• Collaborate with interdisciplinary rehabilitation team; refer to other disciplines/agencies as appropriate.
• Maintain timely clinical and billing documentation and attend required meetings and patient care/staffing conferences.
• Order, maintain, clean, and report equipment needs and repairs.
• Promote and maintain a safe environment; identify and eliminate fire and safety hazards.
• Ensure patients and families receive high-quality, compassionate service.
• Perform other duties as assigned by management.
