Full-time Certified Nurse Midwife opportunity within a Patient-Centered Medical Home (PCMH) model, working collaboratively with physicians and a multidisciplinary care team to provide comprehensive women’s health services.
**Position Overview**
-Full-time opportunity
-Collaborative practice model with supervising physicians
-Comprehensive women’s health services for patients of all ages
-Preventive care, obstetric care, and health maintenance
-Participation in quality assurance and peer review activities
-Team-based care environment
**Clinical Responsibilities**
-Obtain relevant health and medical histories
-Identify medical risks and patient needs
-Order and interpret diagnostic testing
-Formulate diagnoses and implement treatment plans
-Prescribe pharmacologic and non-pharmacologic therapies
-Develop patient education plans
-Make referrals to specialists and community agencies
-Schedule and conduct follow-up care
