Role summary:
Provide physical therapy assessment and treatment in a rehabilitation setting, following established care plans and documentation standards.
Qualifications:
- Bachelor’s degree in Physical Therapy
- Active New Hampshire PT license
- Minimum 1 year of clinical experience as a Physical Therapist
- BLS (AHA) certification
Responsibilities:
- Evaluate patients and develop individualized treatment plans
- Implement therapeutic interventions and monitor patient progress
- Document assessments, plans, interventions, and outcomes per facility policy
- Communicate with interdisciplinary team and patients/families regarding goals and progress
- Maintain compliance with regulatory and clinical standards
